Enrollment of Baby/Addition of Newborn to your medical services plan
Usually, when a child is born in British Columbia, the hospital will provide the parent with a Baby Enrollment Form to enroll the child with MSP. If you are a beneficiary of MSP and need to enroll a newborn, you can use the download the form from Health Services. -
Application for Addition of Family Members
If you are already enrolled in MSP under a self-administered plan and you want to add a spouse or child (other than a newborn) to your account, please print, complete and submit this application. Your spouse or child must be a resident of British Columbia in order to qualify for benefits. Please include, with the application, photocopies of documents that verify the Canadian citizenship or immigration status of each person to be added to your account.
Please note, if you are enrolled with MSP under a plan administered by a third party (for example, an employer, union, pension plan, the Ministry of Human Resources, or Health Canada) you should contact that agency for the appropriate application.
